University of Wisconsin - Stevens Point Athletics

Any UW-Stevens Point student who is intending to participate in varsity athletics must complete all required medical paperwork and pre-season testing prior to participation in any sanctioned sporting event (practice, strength & conditioning, or competition).
 
Front Rush registration is done by each team's coaching staff. Individual login information will be shared with each student athlete upon registration. There are 8 medical forms that must be completed without errors prior to August 1^st 2025. Late paperwork will result in a $50 late fee added to the student's bill.

Sickle Cell Form:
The NCAA requires all student-athletes have sickle cell trait results on file at the institution they are participating in. Many athletes are tested at birth and birth records are accepted. Please discuss sickle cell testing with your provider at your physical to determine the best course of action in obtaining sickle cell trait test results.
 
Returning student-athletes who have test results on file do NOT need to upload test results annually. Initial test results will be kept on file for the duration of their time as a student athlete at UW-Stevens Point.
 
*Sickle Cell testing can be done on-campus for current student athletes with UW-Stevens Point student health services for $19 charged to the student's bill. Appointments can be scheduled at 715-346-4646 during the academic year.
 
Insurance Form:
UWSP has a standardized Insurance Acknowledgement (PDF) form that must be printed and signed by the policy holder of the athlete's insurance. 
 
It is an NCAA requirement that all student-athletes are medically insured for athletic related injuries. A copy of the front and back of the insurance card must also be uploaded into this form. Card details must be clearly legible.
 
Any changes in insurance coverage throughout the academic year must be communicated to athletic training staff.
 
Physical Form:
Annual physicals are required for sports participation at UW-Stevens Point. All physicals must be completed on the UW-Stevens Point Physical Form (PDF).
